Reporting Alleged Copyright Infringement
The Chapel Chronicles respects the intellectual property of others. It is our policy to respond to clear notices of alleged copyright infringement. This page describes the information that should be present in these notices.
To file a notice of infringement with us, you must provide a written communication, by fax or regular mail, not by email, that sets forth the items specified below. Please note that you may be liable for damages (including costs and attorneys’ fees) if you materially misrepresent that material is infringing your copyrights. If you are not sure whether material available on our website infringes your copyright, you may wish to consult an attorney prior to sending a notification.
If you believe your copyrighted work has been copied and is accessible on this website in a way that constitutes copyright infringement, please use the following format for your written notification.
Format for Written Notification of Alleged Copyright Infringement
1. The name, address, and electronic signature of the copyright owner or a person authorized to act on behalf of the copyright owner.
2. Identification in sufficient detail of the copyrighted work you believe has been infringed. For example, “The copyrighted work at issue is ‘My Poem,’ by John Doe, Published by Doe Publishing, ISBN #0123456789101”.
3. Identification of the material that you claim is infringing the copyrighted work listed in Item #2 above and information reasonably sufficient to permit us to locate the material, for example, the URL of the infringing material.
4. Your contact information, including your name, address, telephone number, and email address.
5. Include the following statement: “I have a good faith belief that use of the copyrighted material described in this notice is not authorized by the copyright owner, its agent, or the law.”
6. Include the following statement: “I swear, under penalty of perjury, that the information contained in this notice is accurate and that I am the copyright owner or I am authorized to act on behalf of the copyright owner to enforce the copyrights that are allegedly infringed.”
7. Sign the notice.
8. Send the notice to our designated agent to receive written notification of alleged copyright infringement at the following address:
